POLICIES

Client’s personal information will be protected and not made available to any parties without the express written consent of the client.

 

Client will be required to complete a questionnaire at the time of intake, informing therapist of all medical conditions and providing a detailed description of the issue requiring attention.

 

Payment is to be made at the time of treatment.  Payment options include cash, credit card and personal check.  A receipt will be provided at client’s request.

 

Insurance will not be accepted as a form of payment for treatment. Receipts of payment may be provided to the client, for client to submit insurance claims.

 

Bodywork sessions will begin and end at the times scheduled.  Client will assume responsibility for any session time missed due to late arrival.

 

Cancellation of an appointment by the client with less than 24 hours notice, or the client failing to show for an appointment will require full payment for the session missed. Exceptions will be made in the case of medical emergency, hazardous weather conditions or serious illness.
